                <text>Background: The new SARS-CoV-2 coronavirus affects a large number of people worldwide, and the elderly are particularly affected because of their vulnerability. Thus, the elderly patients and those with comorbidities have an increased risk of developing a severe disease and show an increased mortality rate. Although they may show mild symptoms of illness and low-grade fever in the early days, they may worsen clinically rapidly, requiring ongoing monitoring. Material and methods: The prospective study was performed on a group of 96 patients (mean age 61.41 ± 3.42 years), with a predominance of men, hospitalized in the Holy Trinity Hospital of Chisinau who met the clinical case definition and were laboratory case-confirmed with COVID-19. Patients were clinically and paraclinically investigated according to the WHO Provisional National Clinical Protocol for COVID-19 infection reporting. The data were statistically processed by the Statistics 10 program. Results: Of the 96 patients with COVID-19, 85 (88.54%) reported at least one had a comorbidity. The prevalence of comorbidities was the following: chronic coronary syndromes (40.1%), hypertension (39.7%), diabetes (16.04%), chronic obstructive pulmonary disease (17.3%), malignancy (13.04%), cerebrovascular disease (10.6%), chronic kidney disease (4.3%) and viral hepatitis B (1.8%). Severe cases of the disease were revealed – 58, medium severity – 38 cases. All critical cases resulting in death (7.29%) showed comorbidities with respiratory symptoms, as well as with the onset of acute respiratory failure. Conclusions: The elderly, male gender and the presence of comorbidities in patients with COVID-19 determine the severe course of the disease and an increased mortality rate.</text>

                <text>The novel coronavirus 2019 (COVID-19) infection caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is a global pandemic that requires a multi-faceted approach to tackle this unprecedent health crisis. Therapeutics to treat COVID-19 are an integral part of any such management strategy and there is a substantial unmet need for treatments for individuals most at risk of severe disease. This perspective review provides rationale of a combined therapeutic regimen of selective endothelin-A (ET-A) receptor antagonism and sodium glucose co-transporter-2 (SGLT-2) inhibition to treat COVID-19. Endothelin is a potent vasoconstrictor with pro-inflammatory and atherosclerotic effects. It is upregulated in a number of conditions including acute respiratory distress syndrome and cardiovascular disease. Endothelin mediates vasocontractility via endothelin (ET-A and ET-B) receptors on vascular smooth muscle cells (VSMCs). ET-B receptors regulate endothelin clearance and are present on endothelial cells, where in contrast to their role on VSMCs, mediate vasodilation. Therefore, selective endothelin-A (ET-A) receptor inhibition is likely the optimal approach to attenuate the injurious effects of endothelin and may reduce ventilation-perfusion mismatch and pulmonary inflammation, whilst improving pulmonary haemodynamics and oxygenation. SGLT-2 inhibition may dampen inflammatory cytokines, reduce hyperglycaemia if present, improve endothelial function, cardiovascular haemodynamics and cellular bioenergetics. This combination therapeutic approach may therefore have beneficial effects to mitigate both the pulmonary, metabolic and cardiorenal manifestations of COVID-19. Given these drug classes include medicines licensed to treat heart failure, diabetes and pulmonary hypertension respectively, information regarding their safety profile is established. Randomised controlled clinical trials are the best way to determine efficacy and safety of these medicines in COVID-19.</text>

                <text>The Severe acute respiratory syndrome may be caused by coronavirus disease which has resulted in a global pandemic. Polymorphisms in the population play a role in susceptibility to severity. We aimed to perform a systematic review related to the effect of single nucleotide polymorphisms in the development of severe acute respiratory syndrome (SARS). Twenty-eight eligible articles published were identified in PubMed, ScienceDirect, Web of Science, PMC Central and Portal BVS and additional records, with 20 studies performed in China. Information on study characteristics, genetic polymorphisms, and comorbidities was extracted. Study quality was assessed by the STrengthening the REporting of Genetic Association (STREGA) guideline. Few studies investigated the presence of polymorphisms in HLA, ACE1, OAS-1, MxA, PKR, MBL, E-CR1, FcγRIIA, MBL2, L-SIGN (CLEC4M), IFNG, CD14, ICAM3, RANTES, IL-12 RB1, TNFA, CXCL10/IP-10, CD209 (DC-SIGN), AHSG, CYP4F3 and CCL2 with the susceptibility or protection to SARS-Cov. This review provides comprehensive evidence of the association between genetic polymorphisms and susceptibility or protection to severity SARS-CoV. The literature about coronavirus infection, susceptibility to severe acute respiratory syndrome (SARS) and genetic variations is scarce. Further studies are necessary to provide more concrete evidence, mainly related to Covid-19.</text>

                <text>One of the current worldwide challenges is struggling with the emerging disease of COVID-19. Following the announcement of the outbreak of the disease by the World Health Organization (WHO), numerous measures have been taken in the field of education, treatment and research at the national, regional and international levels by governmental and non-governmental organizations (NGOs). Iran, like other countries in the world, has taken extensive measures in this regard. On Feb. 25, 2020, the National Headquarter for Combating Corona (NHCC) was formed.1 Immediate and necessary decisions and measures such as closing schools and universities were taken and implemented by this headquarter. The initial instructions about the COVID-19 were published by official websites, including the Ministry of Health and Medical Education (MoHME) and the Universities of Medical Sciences included information on disease symptoms, methods of the prevention, diagnosis, and treatment of COVID-19. Another concern about public health was mental and psychological health. Unknown and unpredictable future in the COVID-19 pandemic, along with misinformation and myths, has led to misunderstandings of health messages in the community. Restrictions on travel and quarantine at domestic and international airports in the spring, when most flights take place, have also raised concerns.2 On the other hand, as previous studies have shown, fear of infectious diseases with high prevalence and mortality rate is associated with other psychological challenges such as stigma, discrimination, loss, feelings of fear of getting sick or dying, and helplessness.3, 4 Therefore, following the activities of the NHCC and MoHME, booklets on stress management and coping with anxiety were published. In this regard, a phone line for psychological counseling has been opened in the Ministry of Health and universities of medical sciences. Meanwhile, children are exposed to a lot of news about the disease epidemic. Therefore, publishing some booklets for children was another measure taken by MoHME to reduce their stress and anxiety. Another activity that took place in the first weeks of the illness and after the closure of schools and universities in Iran was teaching the students by distant education using virtual content and software. This kind of education ensured the students and their family and made them calm because they did not need to attend schools, universities or dormitories. In addition to government agencies, NGOs and charity organizations have taken steps to address the physical, economic, social and psychological harms of the disease. For example, some of these groups provided the equipment and supplies needed for patients and even members of the medical team, such as masks, gloves, and gown. These charity groups have also provided financial support to people suffering from financial problems during the closure of their businesses. Despite all the measures taken in various areas at the national and provincial levels by government and non-government agencies, there are still some shortages. Because Iran is under numerous sanctions, especially in the field of health, and due to the lack of resources and equipment for healthcare services, an increase in the morbidity and mortality rate might happen in the long time.5 Although some efforts were made regarding the stress and anxiety of the public health, especially children and youth, there are some other measures which could be applied. Through online counseling, psychologists can provide training on how to deal with health issues caused by domestic conflicts, tensions between children and parents, and anxiety about illness. It is important to note that financial worries, sanctions, pressures, and deficiencies resulting from them could be associated with physical and mental disorders for people in Iranian society, especially for vulnerable, high risk groups such as low-income individuals and their families. Therefore, the support of international community and WHO, at least in lifesaving medical supplies, is essential. These kinds of lifting sanctions could decrease the tragedy and contribute to preventing the second wave of spreading the disease.</text>

                <text>COVID-19 outbreak, caused by severe acute respiratory syndrome (SARS)-CoV-2 coronavirus has become an urgent health and economic challenge. Diabetes is a risk factor for severity and mortality of COVID-19. Recent studies support that COVID-19 has effects beyond the respiratory tract, with vascular complications arising as relevant factors worsening its prognosis, then making patients with previous vascular disease more prone to severity or fatal outcome. Angiotensin-II converting enzime-2 (ACE2) has been proposed as preferred receptor for SARS-CoV-2 host infection, yet specific proteins participating in the virus entry are not fully known. SARS-CoV-2 might use other co-receptor or auxiliary proteins allowing virus infection. In silico experiments proposed that SARS-CoV-2 might bind dipeptidyl peptidase 4 (DPP4/CD26), which was established previously as receptor for MERS-CoV. The renin–angiotensin–aldosterone system (RAAS) component ACE2 and DPP4 are proteins dysregulated in diabetes. Imbalance of the RAAS and direct effect of soluble DPP4 exert deleterious vascular effects. We hypothesize that diabetic patients might be more affected by COVID-19 due to increased presence ACE2 and DPP4 mediating infection and contributing to a compromised vasculature. Here, we discuss the role of ACE2 and DPP4 as relevant factors linking the risk of SARS-CoV-2 infection and severity of COVID-19 in diabetic patients and present an outlook on therapeutic potential of current drugs targeted against RAAS and DPP4 to treat or prevent COVID-19-derived vascular complications. Diabetes affects more than 400 million people worldwide, thus better understanding of how they are affected by COVID-19 holds an important benefit to fight against this disease with pandemic proportions.</text>

                <text>The emergence of the novel coronavirus (SARS-CoV-2) and the worldwide spread of the coronavirus disease (COVID-19) have led to social regulations that caused substantial changes in manners of daily life. The subsequent loneliness and concerns of the pandemic during social distancing, quarantine, and lockdown are psychosocial stressors that negatively affect the immune system. These effects occur through mechanisms controlled by the sympathetic nervous system (SNS) and the hypothalamic-pituitary-adrenocortical (HPA) axis that alter immune regulation, namely the conserved transcriptional response to adversity (CTRA), which promotes inflammation and diminishes antiviral responses, leading to inadequate protection against viral disease. Unhealthy eating habits, physical inactivity, sleep disturbances, and mental health consequences of COVID-19 add on to the pathological effects of loneliness, making immunity against this ferocious virus an even tougher fight. Therefore, social isolation, with its unintended consequences, has inherently paradoxical effects on immunity in relation to viral disease. Though this paradox can present a challenge, its acknowledgment can serve as an opportunity to address the associated issues and find ways to mitigate the adverse effects. In this review, we aim to explore, in detail, the pathological effects of the new social norms on immunity and present suggested methods to improve our physical, psychological, and healthcare abilities to fight viral infection in the context of the COVID-19 pandemic.</text>

                <text>To investigate the prognostic value of a novel immune-inflammatory index, the interleukin-6-to-lymphocyte ratio (IL-6/LY), with the clinical outcomes of severe coronavirus disease 2019 (COVID-19) cases. A cohort study of COVID-19 patients in Tongji Hospital, from January 2020 to February 2020, was evaluated. Kaplan-Meier method and the log-rank test was performed to analyze survival data. Univariate and multivariate analyses were performed with COX proportional hazard regression model. The primary and secondary outcomes were in-hospital mortality and multiple organ dysfunction syndrome (MODS), respectively. Total 320 adult patients were enrolled in our analyses. Patients were divided into low IL-6/LY group and high IL-6/LY group based on the cutoff value with 2.50. The Kaplan-Meier survival curves showed that high-value group (IL-6/LY ≥ 2.50) had a greater risk of poor prognosis (P &lt; 0.001, respectively). Multivariate analysis indicated that IL-6/LY was the independent risk predictor for in-hospital mortality (hazard ratio [HR], 3.404; 95% confidence interval [CI], 1.090-10.633, P = 0.035) and MODS development (HR, 4.143; 95%CI, 1.321-12.986, P = 0.015). Meanwhile, IL-6/LY was positively correlated with the MuLBSTA score (r = 0.137, P = 0.031), suggesting that IL-6/LY was associated with long-term mortality (90-day). Furthermore, kinetic analysis revealed that the dynamic changes of inflammatory immune indexes were related to the severity of the disease. The elevated IL-6/LY was related with the increased risk of poor prognosis. Not only that, IL-6/LY could be used for risk stratification and early clinical identification of high-risk patients.</text>

                <text>The emergence of new pathogenic viral strains is a constant threat to global health, with the new coronavirus strain COVID-19 as the latest example. COVID-19, caused by the SARS-CoV-2 virus has quickly spread around the globe. This pandemic demands rapid development of drugs and vaccines. Plant-based vaccines are a technology with proven viability, which have led to promising results for candidates evaluated at the clinical level, meaning this technology could contribute towards the fight against COVID-19. Herein, a perspective in how plant-based vaccines can be developed against COVID-19 is presented. Injectable vaccines could be generated by using transient expression systems, which offer the highest protein yields and are already adopted at the industrial level to produce VLPs-vaccines and other biopharmaceuticals under GMPC-processes. Stably-transformed plants are another option, but this approach requires more time for the development of antigen-producing lines. Nonetheless, this approach offers the possibility of developing oral vaccines in which the plant cell could act as the antigen delivery agent. Therefore, this is the most attractive approach in terms of cost, easy delivery, and mucosal immunity induction. The development of multiepitope, rationally-designed vaccines is also discussed regarding the experience gained in expression of chimeric immunogenic proteins in plant systems.</text>

                <text>Introduction: COVID-19 has halted the economic and social progression of the human race. This pandemic has exposed the vulnerabilities of all walks of life. But, most of all, this crisis has jolted the health care systems around the globe. A decrease in emergency surgical interventions was observed at District headquarters Hospital, Rawalpindi. The purpose of this study was to evaluate the impact of a pandemic on acute surgical emergency presentation and referral to a tertiary care hospital.  Material and Methods: It is a retrospective cohort study. We compared emergency surgical interventions requiring spinal or general anaesthesia followed by admission in a ward at DHQ hospital, Rawalpindi during a control period (15th March 2019–15th June 2019) and during the pandemic lockdown period (15th  March 2020- 15th June 2020).  Results: A total of 228 cases were included in the study including both groups. About 73% (167 ) cases were performed in an emergency during Pre COVID-19 period i.e. from March 15th- June 15th, 2019.  A total of 41 exploratory laparotomies were performed in a total of which 28 (68%) were done in the control period while 13 (32%) were done during the lockdown period. Out of 13 laparotomies due to road traffic accidents, only 3 were done during the lockdown period. Civilian Violence causing penetrating trauma resulted in 21 laparotomies in total out of which 11 were before the COVID-19 crisis and 10 during the lockdown. A total of 107 appendectomies were performed. Out of which 75 (70%) were performed in the Pre COVID-19 pandemic. Less than half the number (32) of appendectomies were done during the lockdown.  A marked decrease in emergency hernia surgeries was observed. In the Pre COVID-19 time period, 13 emergency hernia surgeries were done, while only 03 surgeries were done during the lockdown. Regarding Hepatobiliary emergency surgeries, none was done during lockdown while 08 were done during three months of the Pre COVID-19 period.  Conclusion: Firstly, Keeping these figures under consideration, surgical units should expect more complicated cases in the coming days and a high influx of patients should be expected once the lockdown is over. Secondly, the question that remains unanswered is that Are we doing unnecessary surgeries other than trauma in an emergency? Thirdly, there is room to consider that all appendicitis and cholecystitis don’t always need surgery. Fourthly, the private sector has the potential to share the burden on public hospitals.</text>
