                <text>Management of critically ill coronavirus disease 2019 (COVID-19) patients remains both risky and technically challenging. A 55-year-old male COVID-19-positive patient with obstructive sleep apnoea (OSA), diabetes, and obesity presented with cough and shortness of breath, escalating to requiring high-flow oxygen therapy by high-velocity nasal insufflation. The patient's flow rate and oxygen fraction remained labile throughout much of the hospitalization. This lability required frequent clinician interactions and use of personal protective equipment. The patient was alert and oriented and was instructed on the operation of the high-flow system, specifically the adjustment of both flow rate and oxygen percentage. The patient was instructed to modify oxygen to maintain an SpO2 (peripheral capillary oxygen saturation) target range, and flow rate to address dyspnoea as well as reduction of flow as tolerated when other staff entered the room. The patient was successfully self-regulated for 10 days and was discharged on 2 L/min nasal cannula on day 14 of his illness.</text>

                <text>Assim como para os anos 1990 a emergência da noção 'agricultura familiar' foi um traço marcante, tanto no debate acadêmico como no campo das políticas públicas, o mesmo acontece na presente década com a chamada 'abordagem territorial' do desenvolvimento rural. Os significados dessa nova maneira de conceber os destinos do espaço rural e as políticas a ele destinadas têm sido explorados em trabalhos de diferentes autores. Neste artigo, pretende-se iluminar um aspecto ainda pouco tratado e que consiste em saber se os moldes em que a disseminação dessa abordagem vem se dando significa um processo de mudança, ou se, diferentemente disso, trata-se de mais um processo em que os termos são incorporados ao vocabulário dos agentes sem a criação de novas instituições capazes de sustentá-la. Essa segunda perspectiva é a que informa a hipótese que guia a exposição e pode ser resumida na afirmação de que a tal movimento corresponde uma 'inovação por adição', na qual pesam elementos típicos daquilo que parte da literatura chama de 'path dependence' ('dependência de caminho'). O artigo faz um breve resgate de como tal abordagem é incorporada no âmbito dos organismos multilaterais dando origem à 'nova visão do desenvolvimento rural' e de como, posteriormente, ela é incorporada no rol de políticas para o rural em países da América Latina. Sob o ângulo teórico, o artigo discute ainda os limites da explicação da mudança pela nova economia institucional, mostrando o que se poderia chamar de 'embeddedness da dependência de caminho'.While the rise of the category 'family farming' was a feature in the 90´s for academic debate and for public policies, the same happens at the present decade with the so called 'territorial approach' to rural development. The meanings of this new way to conceive the destiny of rural spaces and the policies to rural development have been studied by different authors. This article aims at to broach one aspect less emphasized in the literature: to know if the adoption of such approach is a process of institutional change or a process of simple addition of a new term to the vocabulary of planners, without the creation of new institutions capable to support an actual new conception. This second perspective offers the content to our hypothesis and it can be summarized by the following statement: the introduction of the 'territorial approach' is an innovation added to the institutional environment of public policies in a typical process of path dependence, with the remaining of some elements concerning the old vision of rural development. The article brings a brief history of how such approach is adopted by multilateral organizations originating the 'new vision to rural development' and how this 'new vision' is transformed when adopted by Latin-American countries. Under a theoretical point of view, the article discusses the limits of the explaining about institutional change provided by the new institutional economics, offering an alternative vision based on the idea of embeddedness of institutional change.</text>

Esses referenciais darão suporte à análise de experiências de produção de agrocombustíveis por agricultores familiares no noroeste do Rio Grande do Sul. A pesquisa evidenciou que, por meio da organização local/regional, os atores envolvidos impregnam novos usos no território, reforçando relações de poder, ampliando sua capacidade de realizar ações para atender as suas demandas e interesses. Por outro lado, apesar das transformações territoriais geradas e da satisfação dos atores envolvidos para com as experiências, permanecem desafios frente à falta de mão de obra, à saída de jovens e o consequente envelhecimento da população rural, além da carência de tecnologia apropriada à agricultura familiar.</text>

                <text>O presente estudo teve como objetivo analisar as relações que se estabelecem e/ou se acentuam a partir da inserção dos agricultores na política pública de agricultura familiar, especialmente no Programa Nacional de Fortalecimento da Agricultura Familiar (Pronaf). Para realização desta pesquisa de natureza interpretativa, utilizou-se o procedimento de amostragem intencional por tipicidade (MARCONI e LAKATOS, 2010), tendo sido identificados dois municípios gaúchos, Maquiné e Nova Santa Rita. A coleta de dados ocorreu entre os meses de abril e setembro de 2014, com agricultores e agricultoras beneficiários de políticas públicas de agricultura familiar nos dois municípios. Os principais procedimentos de coleta adotados foram a realização de entrevistas semiestruturadas e a confecção de um caderno de campo. A análise do material empírico esteve amparada pelas discussões sobre as relações inerentes à empresa e ao processo de empresarização do mundo (SOLÉ, 2008; ABRAHAM, 2006). A ênfase nos cultivos comerciais em detrimento da agricultura de subsistência, o acesso individualizado aos financiamentos, a proliferação das relações de parceria (uma maneira particular e informal de empregar pessoas), entre outras situações observadas no campo, evidenciam um processo de assimilação e intensificação das relações funcionais, impessoais, de dominação, de assalariamento e de concorrência associado ao acesso às políticas públicas e às relações contratuais com grandes redes varejistas. Todavia, também foram observadas atitudes de agricultores e agricultoras organizados que conseguiram contornar determinados aspectos da empresarização.</text>

Dadas às possibilidades científicas, técnicas e organizacionais, há efetivamente, a possibilidade, também, na produção agropecuária, do processo de produção imediata ser plenamente realizado pela família produtora, sem implicar, necessariamente, ao contrário, perdas de produtividade ou eficiência.</text>

                <text>Este trabalho pretende examinar a partir de evidências empíricas, a relação que frequentemente acaba por se estabelecer entre as configurações socioeconômicas de sistemas produtivos da agricultura familiar com os processos de degradação ambiental observados, sobretudo no uso predatório dos recursos naturais nos espaços rurais, considerando-se as concepções e percepções que os agentes sociais atribuem ao ato produtivo e sua relação com o ambiente. O estudo abordado neste artigo é resultado de uma investigação baseada na experiência entre agricultores em unidades de produção familiar no âmbito do município de Pontes e Lacerda, localizado no Vale do Rio Guaporé, região sudoeste do estado de Mato Grosso e que está situada uma localidade de transição entre os biomas do pantanal mato-grossense, o cerrado e a floresta amazônica brasileira, e ainda junto à fronteira hidrológica das bacias do Prata e Amazônica, onde foram entrevistados 27 agricultores em áreas inferiores a 400 hectares.</text>
