Case Presentation: We report a critical case of COVID-19 in a young woman without any medical history. She was admitted to the hospital with a rare, but serious presentation of temporary complete heart block with a skin rash after three weeks of treatment with an antiviral agent and hydroxychloroquine. The result of cardiac monitoring, using a Holter monitor, was normal, and her sinus rhythm returned to normal without any interventions. The Maryland 27(Md27) strain was first isolated in 1976 from diseased chicken flocks in the Delmarva Peninsula region. To understand the genetic diversity and phylogenetic relationship of existing strains with Md27, the complete nucleotide sequence of the 3’end coding region (~7.2 kb) of Md27 was determined and compared with other IBV strains and coronaviruses. It has the same S-3-M-5-N-3’ gene order, as is the case of other IBV strains. The spike gene of Md27 exhibits 97% identity with the SE17 strain. There are deletions at the spike gene, non-coding region between M and 5 genes, and at the 3’untranslated region (UTR), which is different from Ark-like strains. Phylogenetic analysis and sequence alignments demonstrate that Md27 is a chimera containing different gene segments that are most closely related to the SE17, Conn and JMK strains. In the context of this global pandemic, this article shows the limits of ‘’thinking national’’ and argues that metamorphization of national to cosmopolitan sustainable development is possible on the basis of overcoming national egoism with cosmopolitan survival. The article builds on the “the theory of metamorphosis” proposed by U. Beck (2010). Phenomena are analyzed through the effects of the “arrow of time” (I. Prigogine) and “methodological cosmopolitanism”. The main results are as follows: The risks of COVID-19 have mixed effects on sustainable development. On the one hand, they undermine the traditional approaches towards social security and sustainability, but on the other hand, there is a chance of establishing cosmopolitan medical cooperation in the struggle against viruses by passing from national biopolitics and national structures of sustainable development to cosmopolitan (global concerted) counterparts. Evidence shows that the predictions of these models are affected by high levels of uncertainty. However, the link between predictions and interventions is rarely questioned and a critical scrutiny of the dependency of interventions on model assumptions is missing in public debate. In this article, I have examined the building blocks of compartmental epidemic models so influential in the current crisis. A close look suggests that these models can only lead to one type of intervention, i.e., interventions that indifferently concern large subsets of the population or even the overall population. This is because they look at virus diffusion without modelling the topology of social interactions. Therefore, they cannot assess any targeted interventions that could surgically isolate specific individuals and/or cutting particular person-to-person transmission paths. If complex social networks are seriously considered, more sophisticated interventions can be explored that apply to specific categories or set of individuals with expected collective benefits. In the last section of the article, I sketch a research agenda to promote a new generation of network-driven epidemic models. Additionally, we revised their level of sustainability by calculating an Organic Livestock Conversion Index (OLCI) using 10 indicators and 36 variables. Information was obtained through two means: direct observation and a questionnaire applied to 119 goat farmers of six municipalities of the region. These goat farms principally consist of creole goats that graze in extensive communal rangelands and grasslands in non-arable lands with xerophytic vegetation, and their principal product is meat of adult goats. The goat farms had a moderate level of OLCI (48.0–53.6%) as well as a moderate level of sustainability (46.0–58%) for the goat farms included in this study). Five indicators showed limited compliance of the goat farms, with organic standards. Five showed a high level of compliance. The greater the OLCI value, the greater the sustainability of the goat farms. Increasing the level of compliance of the goat farms of the lower Mixteca region of Puebla, with the organic standards, which would in turn increase their level of sustainability, would require improving technical, social, environmental, and economic aspects of these farms, as well as co-responsibility in promoting sustainable organic goat farming by the different social actors involved (farmers, researchers, technical advisors, and policymakers).
