El presente estudio se realizó en el municipio de Zacatlán, Puebla con el objetivo de caracterizar la calidad de los Andosols bajo diferentes sistemas de manejo. Los métodos que se emplearon fueron: entrevistas a productores para identificar los sistemas, descripción de perfiles de suelos, análisis de laboratorio y análisis de similaridad de los diferentes manejos. Los resultados indicaron que la zona de estudio está integrada por tres sistemas: bosque mesófilo de Montaña (BMM), agroforestal (asociaciones manzana-maíz-calabaza-arándanos) y agrícola, con adición de abonos orgánicos (FO) o fertilización inorgánica (FI). En los Andosols con BMM, la estructura biológica domina (agregados migajosos y granulares hasta los 55 cm de profundidad), presentan alto contenido de materia orgánica del suelo (MOS) (31%), alta retención de humedad (194.5%) y baja densidad aparente (Da) (0.49 g cm-3). Con el cambio de uso de suelo (de bosque a agroforestal o agrícola), la estructura biológica se reduce en espesor o desaparece y dominan los bloques sub-angulares, debido a la disminución drástica del contenido de MOS (a 16.3 o a 6.3%), y retención de humedad (de 97 o a 47%). Además, la Da aumentó hasta 1 g cm-3 en el sistema agrícola. Por el contrario, en tierras degradadas, el sistema agroforestal con manejo orgánico ha mejorado la calidad del horizonte superficial en menos de seis años y con la FO o FI algunos sistemas agroforestales aumentaron los contenidos de Ntot y P. Los índices que mejor caracterizaron la calidad de los Andosols bajo diferentes sistemas fueron: estructura del suelo, MOS, Ntot, P y retención de humedad. These strains and the cocktails obtained from them were used in assays to measure glucose concentration produced from carboxymethylcellulose, crystalline cellulose and filter paper units (FPU). The obtained values were that only the cocktail from Bacillus sp. collected at a “panela” artisan factory produced glucose concentrations that might be of interest, after the cellulases degraded all substrates: carboxymethylcellulose, crystalline cellulose and filter paper. The amount of glucose liquor obtained in the best assay was measured in HPLC, producing 3.45 mg/ml of glucose after the action of endo-beta-glucanase and 1.8 mg/ml of glucose in presence of exo-glucanase. The amount of filter paper units produced in vitrowere 0.0073 FPU. However, the study found that, since the Payback Period for the investment was 6 years, it would not be recommendable to invest in a production plant with these cocktails. Videoconference-based telehealth has been shown to be an effective and acceptable mode of service delivery for exercise-based interventions for chronic knee pain; however, specific training in telehealth is required for physiotherapists to effectively and consistently deliver care using telehealth. The development and evaluation of training programs to upskill health care professionals in the management of osteoarthritis (OA) has also been identified as an important priority to improve OA care delivery. ObjectiveThis study aims to explore physiotherapists’ experiences with and perceptions of an e-learning program about best practice knee OA management (focused on a structured program of education, exercise, and physical activity) that includes telehealth delivery via videoconferencing. MethodsWe conducted a qualitative study using individual semistructured telephone interviews, nested within the Physiotherapy Exercise and Physical Activity for Knee Osteoarthritis randomized controlled trial, referred to as the PEAK trial. A total of 15 Australian physiotherapists from metropolitan and regional private practices were interviewed following the completion of an e-learning program. The PEAK trial e-learning program involved self-directed learning modules, a mock video consultation with a researcher (simulated patient), and 4 audited practice video consultations with pilot patients with chronic knee pain. Interviews were audio recorded and transcribed verbatim. Data were thematically analyzed. ResultsA total of five themes (with associated subthemes) were identified: the experience of self-directed e-learning (physiotherapists were more familiar with in-person learning; however, they valued the comprehensive, self-paced web-based modules. Unwieldy technological features could be frustrating); practice makes perfect (physiotherapists benefited from the mock consultation with the researcher and practice sessions with pilot patients alongside individualized performance feedback, resulting in confidence and preparedness to implement new skills); the telehealth journey (although inexperienced with telehealth before training, physiotherapists were confident and able to deliver remote care following training; however, they still experienced some technological challenges); the whole package (the combination of self-directed learning modules, mock consultation, and practice consultations with pilot patients was felt to be an effective learning approach, and patient information booklets supported the training package); and impact on broader clinical practice (training consolidated and refined existing OA management skills and enabled a switch to telehealth when the COVID-19 pandemic affected in-person clinical care). ConclusionsFindings provide evidence for the perceived effectiveness and acceptability of an e-learning program to train physiotherapists (in the context of a clinical trial) on best practice knee OA management, including telehealth delivery via videoconferencing. The implementation of e-learning programs to upskill physiotherapists in telehealth appears to be warranted, given the increasing adoption of telehealth service models for the delivery of clinical care. Although medical institutions were the most identifiable sources of MERS transmission in South Korea, similar to other countries, in-depth epidemiological investigation was required for some confirmed cases with indefinite contact history or hospital visit records. The subject of epidemiological investigation in the present study was a 35 year-old male patient diagnosed with MERS (#119) who lived in Asan-city and worked in Pyeongtaek-city. Various potential sources of transmission were carefully investigated. While he could have been exposed to MERS through a friend from Saudi Arabia or confirmed MERS cases in his workplace, neighboring areas, and medical institutions, as well as contacts in his home, the chances of transmission were low; however, the potential for transmission through his local community could not be excluded. Practically, it was difficult to determine the modes of transmission for all outbreak cases in communicable disease that occurred in this short period of time. The investigation to identify the mode of transmission in this case was ultimately unsuccessful. However, the various data collected and analyzed to reveal modes of transmission provided detailed information that could not be collected using only interview surveys.
