This study described the epidemiological pattern of viral and atypical bacteria as causes for severe acute respiratory infections (SARI) in hospitalized patients in Egypt. Patients and methods SARI surveillance was carried out at a Teaching University Hospital during the period 2010–2014. All hospitalized adults and pediatric patients meeting the WHO case definition criteria for SARI were enrolled. Nasopharyngeal/oropharyngeal swabs were collected and samples were tested using reverse transcription-PCR for influenza A, B, respiratory syncytial virus, human metapneumovirus, parainfluenza viruses 1, 2, 3, 4, adenovirus, human bocavirus, coronavirus, enterovirus, rhinovirus, and atypical bacteria (Mycoplasma spp., Chlamydia spp., and Legionella spp.). Results Overall, 1075/3207 (33.5%) cases had a viral etiology, and included 912 (84.4%) women and 163 (15.6%) men, with a mean age of 5.74±13.87 years. The highest rates were reported for respiratory syncytial virus (485 cases, 45.2%), parainfluenza virus (125, 11.6%), and adenovirus (105, 9.8%). Single viral etiology was reported in 901 (83.3%), while 174 (16.7%) cases had multiple etiologies. Children had a higher rate (981, 91.2%) compared with 94 (8.8%) cases in adults. Only three and one cases were positive for Mycoplasma spp. and Chlamydia spp. infections, respectively. Neither coronavirus nor Legionella spp. were detected. Conclusion Viral infections were encountered in one-third of hospitalized Egyptian adult and pediatric patients with SARI. Atypical bacteria had a minor role in SARI in our locality. Ongoing surveillance programs will better describe the epidemiology of SARI and will provide specific data to enable decision makers to take appropriate prevention measures. Although remdesivir performed well in preclinical studies, it did not meet efficacy endpoints in a randomized trial conducted during an Ebola outbreak. Remdesivir holds promise for treating COVID-19 based on in vitro activity against SARS-CoV-2, uncontrolled clinical reports, and limited data from randomized trials. Overall, current data are insufficient to judge the efficacy of remdesivir for COVID-19, and the results of additional randomized studies are eagerly anticipated. In this narrative review, we provide an overview of Ebola and coronavirus outbreaks. We then summarize preclinical and clinical studies of remdesivir for Ebola and COVID-19. Patients with severe symptoms of COVID-19 may also present with acute neurological emergencies such as ischemic stroke. Ischemic stroke in these patients may result from COVID-19 related complications or decompensation of previously asymptomatic cerebrovascular disorders, or concurrent ischemic stroke from common stroke risk factors in a patient with COVID-19. Acute ischemic stroke patients with large vessel occlusions require emergent triage, intensive care, and mechanical thrombectomy. Management of patients with large vessel occlusions (LVO) requires special considerations in the current pandemic. Physicians must now account for prognosis of severe COVID-19, resource utilization, and risk of infection to healthcare workers when determining eligibility for mechanical thrombectomy (MT). Here, we describe important prognostic factors including age, laboratory, and imaging findings to consider for MT selection and provide suggestions for taking care of patients with LVO and possible or confirmed COVID-19. It is recommended to perform MT in patients within the established guidelines, and consider a conservative approach in cases where there is clinical equipoise to minimize futile reperfusion. Lastly, we describe an illustrative case of a patient with ischemic stroke and COVID-19. Differences exist in medical education for both countries, allowing South Korean medical graduates to have a more holistic education in comparison to their Italian counterparts, who specialize in medical education earlier on. Additionally, there are fewer South Korean physicians per 1000 people in South Korea compared to Italian physicians per 1000 people in Italy. South Korea employed technology and the holistic education of its physician community, despite having a smaller proportion of physicians in society, to its advantage by implementing efficacious drive-through centers that test suspected individuals rapidly and with little to no contact with healthcare staff, decreasing the possibility of transmission of COVID-19. Conversely, Italy is presently considered the epicenter of the outbreak in Europe and has recorded the highest death toll of any country outside of mainland China. This is partially due to the reactionary nature of Italy’s public health measures compared to South Korea’s proactive response. The different healthcare responses of South Korea and Italy can inform decisions made by public health bodies in other countries, especially in countries across the Americas, which can selectively adopt policies that have worked in curtailing the spread of COVID-19 and learn from mistakes made by both countries. The calculation of the disease-free and the endemic equilibrium points is also performed. The model has the following main characteristics: (a) the exposed subpopulation is infective, as it is the infectious one, but their respective transmission rates may be distinct; (b) a feedback vaccination control law on the Susceptible is incorporated; and (c) the model is subject to delayed partial re-susceptibility in the sense that a partial immunity loss in the recovered individuals happens after a certain delay.
